CA-1 Information
CA-1 forms to be filed in Teacher Education Services
Persons changing rank or adding an area of certification must file a CA-1 with Teacher Education Services. The address is as follows:

Forms are available from your Board of Education, Teacher Education Services, or the EPSB website. Complete pages 1, 3 and the top portion of page 5. Page 1 of the CA-1 must be signed by your superintendent prior to submission to TES. Also, be certain to specify the desired action (e.g., rank change, principal certification) on the CA-1 where appropriate. A certified check or money order for the appropriate amount should be mailed to the standards board and should not be attached to your CA-1 or you may pay online to EPSB. EPSB is responsible for matching your check with your CA-1, so be certain to attach a letter providing adequate information to accomplish this task.
Transcripts
Transcripts must be requested directly through the National Student Clearinghouse.
