Proposal Approval Form (pdf)
This form accompanies a proposal or contract internally and is used to obtain the
appropriate signatures and approvals. The form can be completed online and printed.
The Proposal Approval Form must be submitted to Sponsored Programs at least two weeks
prior to final submission date.
